System Requirements

To complete our on-line form, you must use the following browser: Internet Explorer version 6.0 or newer. To check what version of Internet Explorer you have:

Open Internet Explorer.
From the top menu, select Help > About Internet Explorer.
A dialog box will appear indicating your version of Internet Explorer.

In addition, your browser must be set up to allow cookies. To see if your Internet Explorer browser is setup to allow cookies or to enable cookies on your browser, follow the instructions below:

Select "Internet Options" from the Tools menu.
Click on the "Privacy" tab.
Click the "Default" button (or manually slide the bar down to "Medium") under "Settings".
Click "OK".
